  • Our approach

Children learn best in familiar environments with people they know and trust, so therapy is delivered in your home or community settings like childcare. Sessions use a play-based, family-centred approach where your Team Link works with you to develop strategies you can use throughout daily routines and activities. This means your child can practice new skills many times during the day without needing to attend clinic appointments. The team uses a transdisciplinary model, so your primary contact works across multiple therapy goals rather than you seeing different specialists separately. Therapy is evidence-driven and tailored to your child's unique strengths and goals across home, school, and community settings.

  • Working with families

Your family is a core part of the team and communication is central to how Care Pathways works with you. Your Team Link works closely with you to develop strategies you can use within everyday activities and routines, ensuring you feel informed, supported, and empowered. The provider highly encourages open communication and active participation in your child's care, and welcomes your feedback about what's working and what could be better—you can speak to your Team Link or contact them directly. Your Team Link attends joint visits with specialists so they understand the outcomes and can help you implement recommended strategies in your daily life.

  • Meet the team

The team includes Speech Pathologists, Occupational Therapists, Physiotherapists, Psychologists, and Early Childhood Teachers who work collaboratively in a multidisciplinary model. Your child will be allocated a Team Link—a designated primary contact who knows your child and family best and coordinates communication across all team members. The Team Link meets with you to understand your child and family, is your main point of contact, works with you most often, has the most detailed information about your child's overall service, and regularly reviews progress toward your goals. Other team members provide specialised assessment or advice as needed, and may attend joint visits with your Team Link to ensure everyone understands the outcomes and can help implement strategies.

  • Your first visit

Your first appointment begins with a comprehensive intake interview to understand your child's strengths, challenges, and developmental history. You'll complete a service agreement outlining the supports and services your provider will deliver, along with consent documentation explaining what you're agreeing to. Two practitioners will attend your first appointment so both can begin understanding your child's history and see where your child is developmentally, and you'll have the opportunity to meet two staff members who will be involved in your child's care. This approach means there will be charges to your NDIS package for both staff and their associated travel costs.
