- Home environment and equipment assessments, assistive technology prescription, home modifications, equipment trials and fittings, and goal-focused therapy sessions to improve daily living skills and independence.
- Physiotherapy
- Strength and balance training, mobility and gait assessments, pain management, post-hospital rehabilitation support, and home exercise programs to enhance mobility and reduce falls risk.
- Dietitian Services
- Comprehensive nutrition assessments, individualised meal and supplement plans, malnutrition and weight management support, and chronic condition management through evidence-based dietary advice.
- Functional Capacity Assessment (FCA)
- Comprehensive in-home assessment of daily living skills, mobility, and equipment needs for NDIS planning.
- Home and Living Reports
- Specialist assessments and reports to support NDIS funding for housing and living supports, including Supported Independent Living (SIL) and Specialist Disability Accommodation (SDA).

Working with families
The provider emphasises close collaboration with families and carers throughout therapy. The paediatric occupational therapist brings a warm, engaging, and family-centred approach, working closely with parents, carers, and educators to build confidence and essential life skills. For all client groups, the team works alongside families and care providers to understand unique goals, tailor services to individual needs, and ensure long-term wellbeing. Communication is supported in multiple languages—English, Cantonese, and Mandarin—to ensure families feel heard and understood.

Harmony Path Healthcare is a team of experienced and compassionate allied health professionals including Occupational Therapists, Physiotherapists, and Dietitians, all fully registered with the Australian Health Practitioner Regulation Agency (AHPRA). The team includes specialists in areas such as neurological rehabilitation, paediatrics, mental health, equipment prescription and home modifications. Therapists are fluent in English, Cantonese, and Mandarin, enabling effective communication and culturally sensitive care for clients from diverse backgrounds. Each clinician brings clinical expertise and genuine empathy to deliver evidence-based, person-centred care tailored to individual goals and needs.
