• Our approach

  ConnectAbility's approach is personal, flexible and person-centred, treating each individual as someone with a history and desired future. Support is provided through an innovative model where the organisation works with you and those closest to you to create a schedule of activities and supports tailored to your interests and needs. A dedicated team of Core Support Officers meets with families to discuss goals and develop detailed support plans that guide support workers in their work. The organisation focuses on creating meaningful relationships and true inclusion, helping to address barriers to achieving goals, whether through further education, volunteering, recreational activities or other community participation. Regular check-ins ensure goals are being met and adjustments are made as needed.

• Working with families

  ConnectAbility emphasises working with you and those closest to you to ensure goals are achieved. The Core Support Officer team meets with families to discuss goals and preferences, then develops a detailed support plan in collaboration with you. CSOs regularly check in with families to follow up on how goals are going and identify any changes needed. The organisation may also organise meetings bringing together support workers, mentors and other support people involved in a person's life to ensure consistency and collaboration. This family-centred approach means parents and carers are actively engaged in planning, goal-setting and reviewing progress throughout the support relationship.

• Your first visit

  When you first connect with ConnectAbility, a Core Support Officer reaches out to meet you and your family to discuss your goals and how you would like your support to look. During this meeting, they will listen to your hopes and preferences to understand what matters most to you. Following this, the CSO will work with you to develop a detailed support plan that outlines what support will be provided throughout the week. Regular check-ins are then scheduled to follow up on progress towards goals and identify any changes or new goals that need to be included. The CSO may also organise meetings with your support team and other support people to ensure consistency and collaboration around your goals.
